NHTD/TBI WAIVER PROGRAMS

SMILE now offers two Medicaid Waiver Programs for families seeking an alternative to nursing homes or institutionalized care so their loved one may be cared for safely at home.

These programs allow seniors and differently abled adults to stay in their communities while benefitting from increased independence, self-reliance, and meaningful activities.

The Nursing Home Transition and Diversion (NHTD) Waiver Program is a Medicaid option available to New York residents with disabilities and seniors who are nursing home eligible but choose to live at home.

The Traumatic Brain Injury (TBI) Waiver Program is another Medicaid option that provides services for New Yorkers ages 18-64 who have suffered a traumatic brain injury or stroke and require a nursing home level of care.

NHTD ELIGIBILITY:

  • Be eligible for Nursing Facility Level of Care (LOC)*
  • Be a Medicaid recipient
  • Be 18 years or older, if between 18-64 documentation of a physical disability is required
  • Choose to live in the community rather than a nursing facility
  • Have or find a living arrangement which meets the individual’s needs
  • Be able to be served with the funds and services available under the HCBS/NHTD waiver and New York State Medicaid State Plan
  • Not a participant in another HCBS waiver program

TBI WAIVER ELIGIBILITY:

  • Diagnosis with TBI or other type of brain injury, such as stroke, anoxia, etc.
  • Be eligible for nursing facility Level of Care (LOC)*
  • Be a Medicaid recipient
  • Be 18-64 years old
  • Choose to live in the community rather than a nursing facility
  • Have or find a living arrangement which meets the individual’s needs
  • Be able to be served with the funds and services available under the HCBS/TBI waiver and New York State Medicaid State Plan
  • Not a participant in another HCBS waiver program

WHAT IS NURSING HOME LEVEL OF CARE*?

It is typically defined as requiring assistance with at least 2 Activities of Daily Living (ADLs).

The 6 standard ADLs are:

  • Bathing
  • Grooming/Personal hygiene
  • Dressing
  • Toileting
  • Eating (not including cooking)
  • Transferring (functional mobility – walking, getting in and out of bed or a chair, etc.)

REGIONAL RESOURCE DEVELOPMENT CENTERS (RRDC)

NHTD & TBI Waiver services are administered through a network of Regional Resource Development Centers (RRDC), each covering specific counties throughout the State. To enroll into a New York City NHTD or TBI Waiver Program (Bronx, Brooklyn, Manhattan, Queens, Staten Island) simply call the RRDC at 718-816-3555

AVAILABLE SMILE SERVICES:

SMILE is authorized to provide the following NHTD/TBI Waiver Services:

  • Service Coordination:
    A dedicated SMILE Service Coordinator assists NHTD participants to ensure all required paperwork is completed and filed so, they may successfully access Medicaid Waiver and other qualifying services.

  • Respite:
    Trained SMILE team members provide family or caregivers peace of mind when they need to leave their loved one for short periods of time.

  • Home and Community Support Services (HCSS):
    Personal Care Aides help a participant to safely live at home with personal care assistance and reminders. These tasks can include, but are not limited to bathing, dressing, light housekeeping, verbal prompts for self- medication administration, meal preparation and oversight/supervision. This service is provided in hourly increments and can be approved for up to 24 hours/day, if necessary.
